Without sounding too obvious... Sit up and look ahead. Get someone to always be on the ground when you're schooling so that they can keep reminding you

As lrh says you have to stop yourself doing it. Think "Sit up, look up, and look where I'm going". Make sure both seat bones (you should be able to feel them) are evenly weighted. It sounds like you're collapsing through your inside hip on corners which is causing the lean in. Your legs may also be shooting forwards to allow for the collapse. A good exercise to do when a hip collapses, is to look over the shoulder on the side of the collapsed hip, this will lift the hip up and should rebalance you.
Try riding without stirrups as well. That'll teach you to use your seat properly. If you lean without stirrups to catch you you'll slide off the side. Riding without stirrups is a marvelous exercise to develop balance and the effectiveness of the seat. I regularly have full hour lessons, without stirrups. It hurts afterwards as you have to use so many more muscles, but it's so beneficial and really does help you to develop a good seat. I think it's a very undervalued exercise.

Like everyone has said...look up and sit up I had a friend who used to look down and I cant for the life of me think why you would look down while traveling at speed :/ lol I know its sounds silly but you wouldn't walk through a busy shopping center with your eyes on the floor so why do it when your on the horse? Also remember that horses go where you are looking ;D

Get an instructor to help you & have a lesson really focusing on your position. We all pick up bad riding habits at times! It took me quite a few lessons to stop 'tweaking' my outside rein a few years ago!

Apart from the obvious of looking up and sitting up, try turning your inside shoulder back to the centre of the circle as you turn (imagine theres a circle!!)...this will help your inside leg come on to the girth slightly too and allow your horse to swing smoothly round the corner rather than losing the hindquarters. I was always taught to look between your horses ears. x
