I think Fylde Saddlery will pretty much make whatever you want. I'm sure there must be others that make to order.

i got mine off e bay .really nice bit and way cheaper than the shops. just found french link wilkie's for b.i.n £10.95 + £1.00 postage item number 260354892185. they have all sizes.
