she needs to open her hips and make sure they are really supple otherwise any tension in her thigh/hip will bring her heels up.
when walking/halt get her to lift her knees up and do a circle (like peddling a bike) with each leg then take away stirrups and do some walk and trot (canter if shes able) telling her to let her legs stretch down (but not forcing them down- tension again)
bring back stirrups (making sure they are not too short/long) and do the same, with her weight dripping down through her heels- level is better than down anyway
is she sat up with her shoulders rolled back and on her bum rather than tipping forwards?

Agree with the taking away stirrups - I got told if you can't ride with them then you can ride without them!!

Really depends on how old she is as to how you put it, but actually 'toes up' worked for little very old me.
Honestly taking away stirrups is not good for a little one, you can frighten them badly.
Taking away stirrups should only be done when child is old enough to give you plenty of cheek.

Does the pony take a little more riding forward in canter? I'm wondering if she's pushing with her heels rather than using her lower leg and that's why her heels are coming up?

buy wedge stirrup treads, they work very well too.

In canter some riders can grip with the knees and this in turn causes the heels and lower leg to rise. Encourage to open the hips (as said before) take the knees away and push the weight into the foot (bricks into feet). Hope this makes sense ...

Tell her to keep her weight in her little toe, worked for me as a youngster!

as long as she's not a tiny tot i would suggest lunge lessons on the pony (or a suitable other horse maybe at a riding school)- that way she doesnt need to worry about controling the horse - or even getting it going and can totally work on her position and opening up through the hips and lowering her legs down and round the horse - she can then work safely without stirrups (and reins) and it honestly works wonders for position and balance - trust me i know - it how my boss at my first equine job sorted me out and in all fairness it is only her and one other person (the marvellous instructor peter cook!)that are truly responsible for improvement of my position and riding!
